Our consultation form is processed through Formspree. Please avoid sending highly sensitive medical, financial, or identification information through the website form.

Protect your information

CaliCare Advocates will not call or text you unexpectedly to request your Social Security number, bank information, Medi-Cal number, or other sensitive personal information. If you receive a suspicious request claiming to be from us, do not provide information. Instead, call us directly at (818) 389-4207.
